Because a two-storey extension carries an upper floor and its own roof load down through two storeys of new wall rather than one, foundations need to be sized and specified for that greater load, and the ground floor walls are effectively structural for the floor above in a way a single-storey extension's walls simply aren't. The first-floor structure — new joists spanning between the extension's walls and, in most cases, tied back into the existing house's floor structure — needs careful design where the new upper floor meets the existing one, since floor levels between old and new sections don't always align without adjustment. Roof design is often the most visually significant decision on a two-storey extension: whether the new roof ties into the existing roofline as a continuation of the original pitch, or forms a distinct but complementary roof form, has a real impact on how well the finished extension reads as part of the house rather than a separate block stuck onto the side or rear.
